What is a progress note in therapy?

A therapy progress note is a clinical record documenting what occurred in a session — the client's presentation, interventions the therapist used, clinical assessment, and the plan going forward. Common formats include SOAP (Subjective, Objective, Assessment, Plan), DAP (Data, Assessment, Plan), BIRP (Behavior, Intervention, Response, Plan), GIRP (Goals, Interventions, Response, Plan), PIRP (Problem, Intervention, Response, Plan), and Narrative.
