What is a superbill?

A superbill is an itemized statement a therapist gives a private-pay client after payment. The client submits it to their insurance plan to request out-of-network reimbursement. It lists the provider, the client, each date of service with its CPT code and fee, the diagnosis code, and what was paid.

Is a superbill the same as a CMS-1500 claim form?

No. A CMS-1500 is the standardized claim form a provider files directly with a payer. A superbill is a statement for reimbursement that the client files themselves. This tool produces a superbill, not a CMS-1500.

What has to be on a superbill for therapy?

Plans commonly expect: the practice name and contact information, the clinician name with credential, license number, and NPI, the client name and date of birth, each date of service with its CPT code, a description, and the fee, an ICD-10 diagnosis code from your records, the total charged, and the amount the client paid. Individual plans vary, so the client should confirm requirements with their plan.

Where do the CPT codes in the dropdown come from?

The dropdown lists common behavioral-health CPT code numbers with plain-English descriptions we wrote ourselves, each verified against public CMS billing guidance. You remain responsible for selecting the correct code for the service you provided, and you can type any code manually.

Why does the tool ask me to type the ICD-10 code instead of suggesting one?

Diagnosis comes from your clinical records, not from a billing tool. A lookup that suggested diagnosis codes would sit too close to clinical advice, so this generator asks for the code you have already documented.

Is my client’s information safe in this tool?

Everything runs in your browser. The form never sends what you type over the network, nothing is logged or stored on any server, and no analytics touch the field contents. Refreshing or closing the page erases everything, which is also why you should print or save the PDF before leaving.

Can I save a superbill for later?

Use the print button and choose "Save as PDF" in your browser’s print dialog. The tool itself deliberately stores nothing, in this browser or anywhere else.

Does a superbill guarantee my client gets reimbursed?

No. Reimbursement depends on the client’s out-of-network benefits, deductible, and plan rules. A complete, accurate superbill improves the odds the claim is processed without back-and-forth, but the plan makes the decision.
